1. What "telehealth" means here
Telehealth is the delivery of clinical services using electronic communications — secure messaging, video, asynchronous health-history forms, and (where appropriate) electronic prescriptions — instead of an in-person visit. When you book a free consult through Legacy Wellness, you are connected with a US-licensed provider through our SamaritanMD partnership. Any clinical evaluation, diagnosis, or treatment is handled privately between you and your licensed provider.
2. Legacy's role vs. your provider's role
Legacy Health and Fitness offers coaching services (training, nutrition, recovery, accountability) and arranges access to a free virtual consult with a US-licensed provider. Legacy does not diagnose, prescribe, or dispense. All clinical decisions are made privately by your licensed provider.
3. Risks and benefits
Benefits: convenience, no travel, faster access to providers, ability to reach a licensed clinician from anywhere in the US.
Risks and limits:
- Telehealth is not appropriate for emergencies. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room.
- Without a physical exam, some conditions cannot be evaluated as thoroughly as in person.
- Technology can fail. Messages can be delayed.
- Despite reasonable safeguards, no electronic transmission is 100% secure.
- A licensed provider may decline to provide a particular service if it is not clinically appropriate.
4. Identity verification
You must be at least 18 years old and a US resident. The telehealth platform may verify your identity, age, and address before completing a consult.
5. Health information and HIPAA
Health information you share with your licensed provider — including your health history and any provider correspondence — is collected and stored by the provider's medical-records system, governed by HIPAA where applicable. Industry-standard safeguards (encryption in transit, access controls, vendor due diligence with appropriate Business Associate Agreements where required) are used to protect that information.
6. Use and disclosure of health information
Your provider uses your health information for treatment, payment, and routine healthcare operations as permitted by HIPAA. Information may be shared with fulfillment partners and shipping carriers where clinically relevant. Your health information is not sold.
7. Your rights
Under HIPAA you generally have the right to: inspect and obtain a copy of your health record; request corrections; receive an accounting of disclosures; request restrictions on use or disclosure; request confidential communications; and file a complaint without retaliation. Email support@legacyhealth.fit and we will route the request appropriately.
8. Cash-pay program
The Legacy Wellness coaching side is a cash-pay program. Coaching is billed on the cycle disclosed at intake. Any clinical services arranged with your licensed provider are billed separately under the provider's own terms.
9. Clinical care is between you and your provider
Anything clinical discussed during your consult — including any treatment decisions and any related disclosures — is between you and your licensed provider. Your provider will explain the relevant clinical considerations directly to you and document them through their own intake. Coaching content provided by Legacy is informational and is not medical advice.
10. Right to discontinue
You may discontinue telehealth services at any time. Discontinuation does not affect your right to seek care from another provider in person or remotely.
11. By submitting a consult request
By submitting a consult request through this Site, you acknowledge that you have read this notice and consent to be contacted to schedule your consult. Final clinical decisions are made by a licensed provider during your consult.
